The In-Home Support Provider, will be providing personal care and companionship to an individual with chronic health issues by planning, preparing, serving meals, cleaning, grocery shopping, picking medical prescription from doctors and pharmacies, coordinating various doctor appointments, and other related in-home support provider tasks. Details Skills and Duties: • Administer personal care to a person as needed; • Planning, preparing, and serving meals; • Cleaning, grocery shopping, and other related housekeeping tasks; • Supporting an individual by: o Picking up medical prescriptions from doctors and pharmacies o Coordinating various doctor appointments o Providing personal care and companionship, due to chronic unresolved health issues. Language: • English Education: • Completion of Secondary school education or First Aid Certificate is required Experience: • 2-3 years of experience of working with a person having disabilities • Completion of training program in care of a person with disabilities or in related filed is required. • Home management experience is required Security and Safety: • Criminal record check required Transportation /Travel Information: • Public transportation is available, Own vehicle Personal Suitability: • Initiative, Excellent oral and written communication Work Conditions and Physical Capabilities: • Ability to perform work at employer’s residence to provide support to an individual with chronic illness
