I have Level II (Secret) Security Clearance (includes background and criminal record check) (2020-2021) I made home visits to an elderly woman two days a week for approximately 5 hours. This included making lunch, checking medication had been taken, playing games, putting on music or movies, clearing up lunch dishes and laundry. She had been diagnosed with dementia, hearing loss, diabetes and had limited mobility with a walker. I took her out for occasional scenic drives and lunch. Phone or e-mail communication with the family after each visit was made. (2017-2019) I visited a senior women in the Rockcliffe Retirement Home several times a week. Wrote e-mails to the family who lived out of town after each visit to describe the day and health of the lady. She had dementia, glaucoma and arthritis in her knees. She initially lived in a unit independently and was later moved to the secure floor of the residence. I drove and escorted her to medical appointments outside of the residence. I joined her for meals, movies and special events at the residence. Close observation was made to her health and I alerted staff when extra help was called for such as recurrent UTIs. Stayed with her at hospital when she fell and was taken by ambulance. Took her back to residence when examination was completed. I had a good rapport with staff at the home and I was their first point of contact. She was later moved to a Memory care home in Kanata. Visited there and attended her after a fall and broken hip and subsequent hospice care. (2009 - 2010) Visited 52 year old man during a mental health crisis and treatment at the Royal Ottawa Hospital. Later that year he was diagnosed with cancer; multiple myeloma. He underwent back surgery and stayed at the Ottawa Rehabilitation Centre for several months. I visited 3-5 times a week (2-3 hours) His family lived in British Columbia and were not able to visit often. I was asked to purchase and drop off any personal items he needed. After release from rehabilitation he lived at his apartment. There were chemotherapy and radiation treatments over several months and then hospitalization with pneumonia. I visited at the hospital several times a week. Subsequently I attended when he received end of life care and the family arrived.
