Position Overview We are seeking a dependable PSW (Personal Support Worker) or RPN (Registered Practical Nurse) for a Casual On-Call Community Care Coordinator position. This role involves facilitating workshops, visiting clients for intake assessments, and assisting with onboarding. Ideal for a healthcare professional who enjoys client interaction, community engagement, and flexible scheduling. Key Responsibilities Conduct client intake visits and assessments in their homes. Facilitate community workshops and information sessions as required. Support onboarding of new clients by explaining services, policies, and procedures. Maintain accurate documentation and ensure smooth transitions into care services. Represent the organization with professionalism, empathy, and respect. Qualifications 3–5 years of experience as a PSW or RPN in healthcare, home care, or community work. Current PSW Certificate or active RPN license in Ontario. Strong communication and interpersonal skills. Ability to work independently and adapt to on-call scheduling. Valid driver’s license and reliable transportation is an asset.
