• Reporting to RN, LPN • Assisting resident for personal care and treat residents with empathy, dignity and respect at all times • Provide personal care according to care plan • Attends to residents according to their needs • Inform NOD if there is any physical or mental status of the residents • Monitor any pressure wound and report to NOD as quick as possible.

• Reports to Nursing Service Unit Manager, Registered nurse in-charge • Delivers quality in-person- centered care includes, Assessment, implementing care plans, providing support, Monitoring and Evaluating and Medication Administration as prescribe • Reviewing and checking medical and diagnostic procedures orders. • Implementing admission and discharge process according to hospital protocols • Prepares and assisting inpatients for any upcoming procedures • Coordinates plan of care and collaborates with interdisciplinary care team • Monitoring patient's status, vital signs and report to Attending Physician for any untoward changes condition of the patients. • Attending patient's concern and observe proper chain of command • Performs administrative duties such as documentation, referrals, etc.
